Guido Levi was born in Rome on March 31, 1888.

Brother of Mario, the first news of him playing for Lazio is from 1910. He was a goalkeeper but since in goal Lazio had first Luc Jaggi and then Lorenzo Gaslini, he played occasionally as a defender. His last appearances were in the 1912-13 season where he played the semi-final against Naples. From what we know, he made 16 appearances for Lazio.
A good athlete and cyclist, Guido volunteered as a soldier in the 7th Alpini Regiment and was later promoted to sergeant in the 5th. In 1908 he was declared a deserter and, after turning himself in, was sentenced to four months and demoted to private by the Military Tribunal of Milan.
On May 22, 1915, he was mobilized. On September 6, 1916, he was seriously wounded in combat. After recovering, he was assigned to assault units and, for his heroic conduct, was once again promoted to the rank of sergeant in the 3rd Alpini Regiment of the Fenestrelle Battalion.
At the end of the war, decorated for the 1915–18 war campaign, he settled in Sanremo. During the 1923–24 season, by then thirty-five years old and having briefly returned to Rome, he played a few matches as a goalkeeper with the third team.
He had an immense love for Lazio.
